The table below lists all of the forms that are likely to be used by self-represented debtors, i.e., persons filing bankruptcy without the advice of an attorney.

The majority of the official forms (the petition, schedules, and statements) are available as PDF "fillable" forms, which means you can complete them online.  Local New Mexico forms are available as PDF, Corel WordPerfect (wpd), and Microsoft Word (doc) documents. You will need the appropriate software to view and print these documents.

New!  See  "Unsworn Declaration Under Penalty of Perjury"  (below) - a generic signature page for use with documents requiring debtor's signature when signature block is not otherwise provided on the form.  This generic signature page replaces the former unsworn declaration under penalty of perjury for filing amended schedules.
