The Bankruptcy Clerk’s Office has designed new forms NM LF 902 and NM LF 903, Declaration by Debtor for Electronic Filing, for e-filed petitions. These streamlined forms replace NM Forms 500-7, 502-7, 500-11, 501-11, 502-11, 500-12, 501-12, 502-12, 500-13, 501-13, 502-13, 500-not individual, 501-712C, 502-not individual, and 501-11C. This reduction of forms should simplify the task of e-filing. The new form may be used immediately, but is required for use as of August 22, 2011, pursuant to NM LBR 9009-1 . The new form can be used for any chapter and debtor type, is available in PDF, WordPerfect, and Word formats, can be found on our website at General ➝ Forms ➝ Debtor’s Signature Pages, and has been provided to the bankruptcy software vendors most attorneys use. Nevertheless, if you use a commercial software package to manage the e-filing of documents, please contact your vendor to ensure the form is available for use by the effective date.
