Instructions for Using the Fillable PDF Reaffirmation Documents

The fillable PDF file for the Reaffirmation Documents is an Acrobat Portfolio containing two pdf files:

  • the fillable coversheet (Official Form B27) and
  • the fillable reaffirmation agreement (Director's Procedural Form B240A).

When you click on the link, you will see a folder named "reaffirmation documents."    Double-click on the folder icon to open the folder.  You will then see two pdf files:  reaff_agreement.pdf and reaff_coversheet.pdf.

Double-click on the coversheet file to open it.  The fields are highlighted. Click on the dropdown arrow in the first field to scroll down and select District of New Mexico.  Tab to go from field to field.  You must enter numbers in fields noted with $ or % symbols.  The coversheet is two pages long.  At the bottom of the second page, click on "Save As..." to save the Portfolio or click on File > Save File from Portfolio, in the Adobe menu bar, to save just the cover sheet file.  Save it with a new file name so you can identify the completed coversheet.  In the District of New Mexico, the debtor's signature must be on every paper verified by the debtor.  Therefore, you will need to print the coversheet to paper, have the debtor sign it, and then, scan the coversheet and save it as a pdf file.

Double-click on the agreement file to open it.  The fields are highlighted. Click on the dropdown arrow in the first field to scroll down and select District of New Mexico.  Tab to go from field to field.  You must enter numbers in fields noted with $ or % symbols.  The coversheet is 7 pages long, but only pages 1 -4 need to be filed with the Court (the rest of the pages are disclosures, instructions, and definitions).  At the bottom of page 4, click on "Save As..." to save the Portfolio or click on File > Save File from Portfolio, in the Adobe menu bar, to save just the cover sheet file.  Save it with a new file name so you can identify the completed coversheet.  As with the cover sheet, the debtor must sign the reaffirmation agreement.  Print the agreement to paper, have the debtor sign it, and then, scan the agreement and save it as a pdf file.

Once you have saved the signed documents as two separate pdf files, you will need to open the coversheet pdf file, and using the Adobe Document menu, click on Insert Pages > From File, select the agreement pdf file, click on Open, and insert the agreement after page 2 of the coversheet.  Save the pdf file.  Now you're ready to e-file the document.
