You may file a proof of claim in paper.  Complete the Proof of Claim and mail the completed form along with an extra copy and a self-addressed, stamped envelope, to the Clerk's Office.   When you submit an extra copy and a self-addressed, stamped envelope, the Clerk's Office will return a "filed" copy to you for your records.  If you choose to deliver the proof of claim to the Clerk's Office in person, please bring an extra copy with you.

Creditor Mailing Lists

The creditor mailing lists for each case as of June 11, 2010, are provided below from the CM-ECF System.  If you see your name listed with an incorrect address, you may file a change of address with the court.  If you do not see your name listed, you may file a request for notice with the court.  See instructions below.

If your address is listed incorrectly on the mailing lists above, or you need to change it for any reason, you may file a change of address.  Just complete the change of address form for either or both cases:
