NM LF 902A abolished

Effective immediately, NM LF 902A – Non-Individual Debtor’s Omnibus Signature Page and Declaration will no longer be used.  Instead, non-individual debtors must use Official Form 202 – Declaration Under Penalty of Perjury for Non-Individual Debtors, which requires the signature of the individual authorized to act on behalf of a non-individual debtor to file documents that require a declaration, and any amendments.  

A non-individual debtor may include the Petition (Form 201) and the Statement of Your Financial Affairs for Non-Individual Debtors Filing for Bankruptcy (Form 207) on Official Form 202 under the category, “Other document that requires a declaration.” Otherwise, the authorized representative of a non-individual debtor must sign the Petition and the Statement of Financial Affairs in addition to Official Form 202.
